If you make a payment in my clinic your card information is not held by me, it is collected by a third party payment processors, who specialise in the secure online capture and processing of credit/debit card transactions.

· To refer you onto a Consultant, where you have given your consent

Livewell Physiotherapy has a debt recovery process in place whereby I appoint a third party to act on our behalf: only relevant information will be shared pertaining to your appointment dates and outstanding amount. I will also share your contact details so they are able to get in touch with you. I will exercise my statutory right to claim interest (at 8% over the Bank of England base rate) and compensation for debt recovery costs under the Late Payment legislation if I am not paid according to our agreed credit terms.

The accuracy of your information is important to me. If any of your details change then please contact me directly on email@example.com.
