Before your initial appointment you must present: Your up to date membership number and pre injury pre-authorisation number from your insurance company. Your policy limitations (i.e excess amount and the amount of money or the number of sessions that has been approved. You also may need a GP or specialist referral prior to commencing treatment.

Physiotherapy treatment recognised and registered with AXA PPP, Aveva, Cigna, WPA, Vitality Health, Simply Health
Please consult with your insurance company directly if treatment is eligible under the terms of your current insurance policy. Please be aware that it is your policy and Livewell Physiotherapy cannot be held responsible for any costs incurred so please contact your insurer first
Payment for treatment is required at the end of each session. If your insurance company rejects to pay an invoice to Livewell Physiotherapy, sent on your behalf, you will be liable to cover any outstanding monies.
Any appointments you undertake prior to advising Livewell Physiotherapy of your wish to use your insurance company OR obtaining your insurance companies approval/full details will be charged to you at the full rate and will not be refundable, in full or in part, once you have approval
Repeated cancellation of physiotherapy appointments, not attended or rescheduled appointments will be charged at the full fee if the client fails to give 24 hours’ notice of non-attendance.
